Professor Sidney Dekker lives in Australia, where he runs the Safety Science Innovation Lab. He is also Professor (Hon.) of psychology at The University of Queensland, and Professor (Hon.) of human factors and patient safety at Royal Children's Hospital in Brisbane. Previously, he was Professor of human factors and system safety at Lund University in Sweden. After becoming full professor, he learned to fly the Boeing 737, working part-time as an airline pilot out of Copenhagen. He has won worldwide acclaim for his groundbreaking work in human factors and safety, and is best-selling author of, most recently, The Field Guide to Understanding ‘Human Error’ (2014), Second Victim (2013), Just Culture (2012), Drift into Failure (2011), and Patient Safety (2011). His latest book is Safety Differently (2015).

Dr Jason Edwards is a researcher with theat Queensland University of Technology's 'Centre for Accident Research and Road Safety – Queensland' (CARRS-Q) . With an academic background is in Psychology, he conducted his PhD in Organisational Psychology and Safety. This PhD project consisted of a three year investigation of the culture of the Australian Heavy Vehicle Industry and the influence this culture has on safety. Dr Edwards has been involved in a number of other research projects both in Australia and in Oman, and has presented at domestic and international conferences. In addition to his research roles, Dr Edwards supervises PhD students and has served as a peer-reviewer on a number of well-respected scientific journals. Jason has a particular interest in applied research which results in real world application and change, and in communicating research findings to non-researchers. His research areas have included reviews of policing policy and practice, examination of heavy vehicle safety and safety culture, traffic safety culture and investigating effective practices for workplace health and safety inspectors.

Andrew is a Chartered Chemical Engineer and RPEQ (Registered Professional Engineer Queensland) with 20 years of engineering and operations experience in the coal seam gas, steel, pulp and paper and water treatment industries. His areas of expertise include Process Safety, Consequence Modeling, Formal Safety Assessments, Risk Assessments, Control of Major Accident Hazards and Safety Cases. His knowledge of toxic, flammable and combustible hazards and the associated safeguards contribute to fit for purpose solutions. Andrew has a range of processing plant and operations experience from working as an operations process engineer where he was part of operations support, shutdown, plant upgrade and commissioning teams. John Green has worked in the oil, gas, petrochemical, electronic and aviation sectors. He began his working life as a major hazards inspector for the HSE in the late 1970s. He then spent periods overseas with BP on Das Island, ADNOC in Abu Dhabi, CALTEX in Bahrain and Total where he was Head of Safety during the construction of the South Pars industrial complex. On returning to the UK John held senior positions with Motorola, British Airways and Board level positions with Severn Trent and Alfred McAlpine. He is currently Director HSEQ and Corporate Responsibility for Laing O’Rourke Australia having held the same post in Europe for 5 years
John holds degrees in Occupational Health and Safety, Integrated Pollution Mgmt, and Risk Mgmt as well as qualifications in Psychology, Law and Change Mgmt. He spends any spare time that he has climbing, scuba diving and in any other pursuit that requires risk management skills.

Maureen Hassall is Associate Professor - Risk at The University of Queensland and is project manager of UQ R!SK an university initiative. Her areas of research focus on developing and applying practical and effective innovative solutions that address the risk challenges faced by industry. Maureen also provides risk management and human factors advice, education and training to industry and at the university. She holds bachelor degrees in engineering and psychology, a MBA and PhD in Cognitive Systems Engineering. Prior to joining UQ, Maureen worked for more than 15 years in mining, processing and manufacturing industries in Australia, New Zealand and Canada.
Jan Hayes

Dr Hayes has 30 years’ experience in safety and risk management. Her project experience covers a range of industries including offshore oil and gas, onshore petrochemicals, gas pipelines, mining, manufacturing and transportation (including air traffic control). She began her career in 1983 in oil and gas process plant operations, and then moved to detailed design. She was involved in the initial Australian oil industry response to the Piper Alpha incident in 1988 and has been working almost exclusively in industrial safety since then. Jan was a director and part owner of Qest Consulting Pty Ltd (one of Australia’s best known industrial risk and safety consultancies) between 1991 and 2004. Holding a number of operational roles, she managed up to 50 staff and was personally responsible for projects ranging from technical safety studies (such as safety cases, HAZOP studies, QRAs and reliability studies) to organisational psychology reviews and human factors studies.
Her current activities cover academia, consulting and regulation. She holds an Associate Professor appointment in the Centre for Construction Work Health and Safety Research at RMIT University where her research interests include operational decision making, safety in design, professionalism and the use of standards. She is Program Leader for the social science research activities of the Energy Pipelines Co-operative Research Centre. In addition, she consults part time with a small group of clients on safety performance improvement projects. Dr Hayes is a member of the Advisory Board of the National Offshore Petroleum Safety Authority.
Dr Hayes holds a Bachelor of Engineering from the University of Adelaide and a Master of Business from Swinburne University and a Doctor of Philosophy in Sociology from the Australian National University.

Amanda Holland has thirteen years experience with an extensive background in Occupational Health and Safety within the oil and gas, mining and construction industries. She holds a Bachelor of Science Health and Safety Management and a Master of Human Factors and Safety Management Systems. Areas of specialty include Auditing, Training and Incident Investigation. Amanda has been involved in the implementation of two behavioural based safety systems with multinational companies and has recently completed a review of her current employers health and safety management system. Her current role is Senior Health and Safety Advisor for Woodside Energy based in Perth, Western Australia.

Trish Kerin

After graduating from RMIT University with honours in mechanical engineering, Trish spent several years working in design, project management, operational, safety and executive roles for the oil, gas and chemical industries.
Trish’s passion for process safety saw her take on several advisory committee roles. This includes representing the Plastics and Chemical Industries Association (PACIA) on drafting the 2007 OHS regulations for major hazards in Victoria – Occupational Health and Safety Regulations 2007. She has also represented the Australian Chamber of Commerce and Industry (ACCI) on the Safe Work Australia temporary advisory group for drafting the major hazard facility model law. Trish has also represented PACIA to the WorkSafe Victoria Major Hazards Advisory Committee and been an invited speaker for various local and international conferences.
She currently sits on the board of the Australian National Offshore Petroleum Safety and Environmental Management Authority (NOPSEMA) and is a member of the Mary Kay O'Connor Process Safety Center steering committee.
Additional appointments have included positions as a director and vice president for a not-for-profit community radio station.
Trish is a Chartered Engineer, a registered Professional Process Safety Engineer and Fellow of IChemE. She also holds a diploma of OHS and is a graduate of the Australian Institute of Company Directors (GAICD).
Recent awards include the Engineers Australia – John A Brodie Medal – for best paper at theAsian Pacific Confederation of Chemical Engineering Congress 2015 (incorporating Chemeca). The award recognises Trish’s peer reviewed paper on Process Safety Competency.

Shoena had worked as a chemical engineer for 12 years when there was a major incident at her place of work that took the life on one employee and injured five others. For the next two years she was involved with the investigation, the decommissioning, and ultimately the demolition and rehabilitation planning for the facility. This experience prompted a change in career direction. She joined the government in 2009 and now leads the team administering the major hazard facility regime under the Work Health and Safety Act in Queensland.

I became Managing Director of GPA in 2011, now employing approximately 150 engineers with offices in Adelaide and Brisbane.

Roberta joined Clough in August 2013 in her current role to support the design, development and implementation of Clough’s HSSE improvement strategy. Significantly this has included developing the Clough Major Accident Prevention Program to address high risk construction activities undertaken by Clough.
Prior to joining Clough Roberta was the HSE Manager for the FMG Solomon Operational Readiness team which included the introduction of autonomous hauling strategy for mining at Solomon. Peter Thorning is Manager, Research Partnerships at Workplace Health and Safety Queensland (WHSQ) within Queensland’s Office of Industrial Relations. A key aspect of Peter’s role is to facilitate access to research so that WHSQ’s regulatory activities have a good evidence base.
Peter was previously Manager, Leadership and Culture and Manager of the Transport Strategy Group. Prior to joining the work health and safety regulator in 2010, Peter worked for the environment regulator, managing the National Pollutant Inventory in Queensland.
Peter has qualifications in occupational health and safety and environmental science. He has a long history of working with industry in Queensland using innovative programs to improve health, safety and environmental outcomes.
Peter is particularly interested in moving beyond compliance with legislative requirements and refocussing attention on reducing harm and achieving outcomes. He is currently undertaking a PhD at Queensland University of Technology which is examining the concepts of Safety-II (that is, making things go right more often) and implications for work health and safety regulation.
